Credentialing Process Overview
Below is a step-by-step outline of what to expect. Each step must be completed in sequence to keep your application moving forward.
Before you begin, download the complete checklist of required forms, documentation, and identification items needed for credentialing. This checklist will help you prepare all necessary materials in advance and avoid delays during the Modio process.
Once your onboarding process begins, you will be provided with an MPAS Ohio email address. This email will be used for all credentialing communications and required documentation throughout the process.

Step-By-Step Outline of What to Expect
Step 1: Set Up and Complete Your Modio Profile
MPAS Ohio uses Modio as the primary credentialing platform. Our credentialing team will send a secure link and instructions to your MPAS email address on file so you can access your profile. Completing your profile thoroughly will help ensure your applications are processed as efficiently as possible.
Please have the following information available before you begin:
- Medical license(s)
- DEA registration
- Education and training history
- Employment history
- Professional references
- Supporting documentation
This information will prepopulate required forms and hospital applications.
If you do not yet have a DEA number or an Ohio medical license, you may temporarily enter 123456789 to proceed within Modio. This is common for recent graduates, providers awaiting licensure, or out-of-state applicants.
You must update this information once your DEA or Ohio medical license is issued.
Completion Deadline:
Your Modio profile should be completed within 5 days of receiving the invitation.
Use the “Save & Next” function to ensure your progress is recorded. You are not fully credentialed after completing Modio. Additional steps follow.
Step 2: Complete Fingerprinting, TB Blood Test, and Notary Forms
Before OhioHealth can process your application, several compliance items must be completed.
TB Blood Test
A TB blood test is required. Skin tests are not accepted.
- If you are located in Central Ohio, you may use OhioHealth Associate Health and Wellness.
- If you are located outside Central Ohio, you may use Jason Health; reimbursement will be processed on your first day of employment.
- You may choose to complete testing at another location; however, these services will not be reimbursed.
Results will be provided directly to you and must be uploaded to Modio. Due to HIPAA regulations, MPAS cannot access your results.
Your application will remain on hold until TB documentation is received.
Fingerprinting
Fingerprint screening is required prior to hospital processing and must be completed in accordance with OhioHealth guidelines.
For Ohio Residents
Ohio residents must schedule fingerprinting through FastFingerprints using the OhioHealth access code.
Access Code: 12KEV2S8
When prompted, confirm that your code was provided by OhioHealth.
You will select a location, schedule your appointment, and enter your applicant information directly through FastFingerprints.
Bring a valid government-issued photo ID to your appointment, such as:
- Driver’s license
- State ID
- Passport
- Military ID
- Green card
Fingerprint results are sent directly to OhioHealth. Please follow all instructions carefully to avoid delays in processing.
For Non-Ohio Residents

Notary Forms
- Complete with a licensed notary.
- Upload finalized documents to Modio.
Step 3: Provide and Verify Your CAQH Information
CAQH is required for credentialing and must be completed or updated before your application can move forward.
If You Already Have a CAQH Profile
Please ensure your profile is current and attested, and that all practice locations are accurate.
You will also need to provide MPAS with access so our credentialing team can review your profile, add required information, and assist with any updates.
If You Do Not Have a CAQH Profile
MPAS can assist with creating your CAQH account to ensure all required information is entered correctly.
Simply indicate this in the form, and our credentialing team will initiate the setup process on your behalf.

To securely provide your CAQH information, please complete the form below:
- Name
- Do you have a CAQH account? (Yes/No)
- CAQH ID (if applicable)
- Username
- Password
This form is encrypted and securely submitted directly to the MPAS credentialing team.
If you have questions or need help completing your CAQH profile, our credentialing team is here to assist you.
Step 4: Review and Submit Credentialing Applications
Once your profile is complete, the MPAS Credentialing Team will generate your hospital applications, typically within one week.
Applications will be emailed to you for review and signature, along with any requests for additional information if needed. Please review all prepopulated details carefully, including dates, references, and work history, before submitting.
Timely responses to any follow-up requests will help avoid delays in processing.
Step 5: Hospital Review & Facility Communication
After submission, OhioHealth and affiliated facilities will review your application.
You may be contacted directly by the facility for additional information. Please respond as soon as possible.
MPAS coordinates submissions, but final credentialing decisions are made by the facility credentialing committees.
Step 6: Payer Enrollment & Insurance Documentation
Once hospital credentialing is underway, the MPAS Credentialing Team will initiate your insurance and payer enrollment.
Enrollment documents will be emailed to you for review and signature, along with any requests for additional information if needed. Please review and return all documents promptly to avoid delays.
Payer approval must be finalized before your employment with MPAS can begin.
Your Credentialing Timeline
Credentialing timelines are structured and deadline-driven. If you anticipate travel, relocation delays, or limited availability, notify the MPAS Credentialing Team immediately. Missed deadlines, incomplete documentation, or delayed responses may impact your start date.
