No. Please submit all required documentation to the MPAS Credentialing Team through Modio or by contacting credentialing directly. We coordinate submissions with OhioHealth and affiliated facilities to ensure proper review and processing.

Yes. Even within the OhioHealth system, a new application must be submitted for each hospital location. The receiving facility will review your application and obtain approval from its credentialing committee.

No. Submission of vaccine documentation is optional with MPAS and OhioHealth at this time.

Notary services are typically available at public businesses such as banks, libraries, tax offices, and similar locations. You may search online for notary services near you to identify the most convenient option.

We encourage providers to use OhioHealth Associate Health and Wellness whenever possible for TB testing. For questions regarding reimbursement eligibility, please contact the MPAS Credentialing Team directly.
